This dataset aggregates and displays the number of New York State Medicaid enrollees by eligibility year and month within each NYS Economic Region, health insurance plan information, and enrollee demographics.

Since 1998, the New York City Police Department (NYPD) has been tasked with the collection and maintenance of crime data for incidents that occur in New York City public schools. The NYPD has provided this data to the New York City Department of Education (DOE). The DOE has compiled this data by schools and locations for the information of our parents and students, our teachers and staff, and the general public.
In some instances, several Department of Education learning communities co-exist within a single building. In other instances, a single school has locations in several different buildings. In either of these instances, the data presented here is aggregated by building location rather than by school, since safety is always a building-wide issue. We use “consolidated locations” throughout the presentation of the data to indicate the numbers of incidents in buildings that include more than one learning community.

The Nursing Home Cost Report (RHCF) is a uniform report completed by New York nursing homes to report income, expenses, assets, liabilities, and statistics to the Department of Health (DOH). Under DOH regulations (Part 86-2.2), nursing homes are required to file financial and statistical data with DOH annually. The data filed is part of the cost report and is received electronically through a secured network. This data is used to develop Medicaid rates, assist in the formulation of reimbursement methodologies, and analyze trends.

The Statewide Planning and Research Cooperative System (SPARCS) Inpatient De-identified File contains discharge level detail on patient characteristics, diagnoses, treatments, services, and charges. This data file contains basic record level detail for the discharge. The de-identified data file does not contain data that is protected health information (PHI) under HIPAA. The health information is not individually identifiable; all data elements considered identifiable have been redacted. For example, the direct identifiers regarding a date have the day and month portion of the date removed.

The dataset contains Potentially Preventable Visit (PPV) observed, expected, and risk-adjusted rates for Medicaid beneficiaries by patient county beginning in 2011. The Potentially Preventable Visits (PPV) obtained from software created by 3M Health Information Systems are emergency visits that may result from a lack of adequate access to care or ambulatory care coordination. These ambulatory sensitive conditions could be reduced or eliminated with adequate patient monitoring and follow up.

Note: The source database for this dataset has been decommissioned. FDNY is currently engaged in creating a new system, but no new data will be added to this dataset.

Potentially Preventable Complications (PPC), obtained from software created by 3M Health Information Systems, are defined as harmful events or negative outcomes that develop or occur during hospitalization and may result from processes of care and treatment rather than from natural progression of the underlying illness.

The PPCs were calculated using Statewide Planning and Research Cooperative System (SPARCS) inpatient data.

This data set includes elevator inspection and report filing information through the New York City Department of Buildings' DOB NOW: Elevator Compliance Filings module. The data is collected because the Department of Buildings tracks elevator inspection information.This data include items such as device number, device status, report filing dates, inspetion dates, BIN, and other location information.

The rates were calculated using Medicaid inpatient and outpatient data for the numerator and Medicaid enrollment in the county or zip code for the denominator.

The observed, expected and risk adjusted rates for PPV are presented by either resident county (including a statewide total) or resident zip code (including a statewide total). This data set includes violations issued on devices through the New York City Department of Buildings' DOB NOW: Safety Violations module. The data is collected because the Department of Buildings tracks violation issuance and related information. This data include items such as violations number, violations type, violation issuance data, and BIN.

The Safe Streets for Seniors program is an initiative aimed at increasing safety for older New Yorkers. Based on factors such as senior population density, injury crashes, and senior trip generators, DOT has selected and studied Senior Pedestrian Focus Areas.
